DESCRIPTION:
The Caswell County Health Department has an opening for an Advanced Practice Provider, to work in the clinic. This position provides direct patient care to a wide variety of clinical programs in coordination with a multidisciplinary team (Providers, Nurses, Social Workers, Nutritionist, etc.)
Work is performed independently, under a collaborative practice agreement with the Medical Director. This position reports to the Health Director in collaboration with the Clinic Director.
Participation in a drug screen and background check are required.

Direct Services
Conducts histories, physical assessments, and case management
The NP coordinates patient care with the help of PHNs who provide
o Patient Education
o Measure vital signs
o Give nebulizer treatments
o Perform lab work as ordered
o Arrange for referral of patients
o Accurately code and document patient visits
Other
Attend meetings and educational conferences/workshops as recommended by the supervisor and adhere to agency policies related to education requests
Represent the agency on community boards and committees as assigned by the Health Director.
Participate in staff meetings
Public Health is a first responder agency for natural disasters (e.g. hurricanes, tornadoes, floods, winter storms), naturally occurring infectious disease outbreaks (e.g. influenza, SARS), technological hazards (hazardous materials releases, critical infrastructure disruptions), and terrorist incidents. This position, like all other positions within the Health Department may be required to participate in emergency response activities as deemed necessary by the Health Director or her designee. Availability during emergencies and exercises is required.
TYPICAL QUALIFICATIONS:
Thorough knowledge of techniques of interviewing, medical and relevant history taking,
Thorough knowledge of principles and practices of general medicine and disease states as they relate to the area of work
Considerable knowledge of health appraisal tools, laboratory tests and findings, advanced physical assessment, advanced pathophysiology, advanced pharmacology and therapeutic procedures as related to the area of work;
Considerable knowledge and ability to plan a regime of care based on assessment and findings
Working knowledge of available resources and appropriate referral methods.
Skill in performing clinical functions.
Ability to examine patients and detect abnormalities
Ability to record accurately and completely all information necessary to evaluate and plan care and to convey this to the physician
Ability to communicate orally
•Ability to teach and counsel individuals, families, and groups in areas of health maintenance, preventive medicine, and care of the sick
Ability to establish and maintain effective working relationships with staff, and to gain the confidence of patients and families and work effectively with them.
Work requires judgment in the application of accepted patient care protocols and medical guidelines in the area
of assignment to determine the appropriate course of action.
License to practice as a Nurse Practitioner
CPR Certification
Employees who are required to drive in the performance of their job duties must show proof of a valid driver's
license.
MINIMUM QUALIFICATIONS:
License to practice as a Nurse Practitioner
SPECIAL REQUIREMENTS:
Possession of or ability to obtain a valid Driver’s License for the type of vehicle or equipment operated.
All offers of employment are conditional upon successful clearance of a background check and pre-employment drug testing. Employment cannot commence until the employee has been cleared.
May be required to possess additional certification(s) as deemed necessary by the Health Services Coordinator.

PHYSICAL REQUIREMENTS AND POSSIBLE HAZARD
Must be able to physically perform the basic life operational support functions of standing, walking, stooping, crawling, fingering/typing, talking, sitting, bending, kneeling, climbing, grasping, hearing, handling, pushing, pulling, lifting, carrying, reaching and repetitive motions of the hands/wrists/feet.
Must be able to perform sedentary work, exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or consistently to lift, carry, push, pull, or otherwise move objects.
Must possess the visual acuity to prepare and analyze data and figures, perform extensive reading, and operate a computer terminal.
Must be able to be exposed to chemicals, contagious and/or infectious diseases, and blood-borne pathogens.
May be subject to possible hazards that include emotional stress due to the nature of interactions with clients or service providers, irate clients and/or family members who may be hostile, resistant, or violent.
